Tourist Visa
Valid passport with at least six months of validity from the date of entry into Indonesia, which has at least one blank page for visa (amendment and endorsement pages cannot be used for visa). A limited-validity passport will not be accepted without a prior Indonesian visa.
Complete and Sign One Application Form. The form must be completed online.
Two recent color photographs (passport size) with white background on picture-quality paper. Note: Do not staple or affixed the photographs on the form.
Copy of inbound and outbound travel itinerary or a letter from travel agents, airline, steamship company, confirming the purchase of tickets.
Letter of employment, indicating the employment status of the applicant. For retiree, please submit proof of the retirement fund. For students please provide letter of intent from the school stating that the student still enrolled at the school.
Copy of most recent bank statement, indicating the current applicant’s bank account in the last two weeks, or a letter from the bank stating that the applicant has a sufficient fund.
Inoculation is recommended but not mandatory, and is required only if applicant had previously traveled to areas/ countries infected by yellow fever.
Provide a list or record of travels in the last 14 days.
Please provide a copy of US Permanent Residence/Visa/Valid I-20 from schools/universities.
Applicants under 18 years of age should attach a notarized copy of birth certificate. In addition, a notarized letter of consent signed by non-traveling legal guardian must be attached if the applicant under 18 years of age travels without the legal guardian.

Business Visa

A business letter from US Company (click here for sample letter) is required, to be typewritten on your company letterhead and addressed to:

The letter must explain all of the following:
Nature of business to be performed
Name and address of reference to be visited
This letter should request Multiple Entries visa if required
Guarantee of return transportation and sufficient funds for the visit
The letter should be signed by an officer of the company other than the applicant.
Invitation letter from counterparts in Indonesia, describing the purpose of the visit.
Copy of inbound and outbound travel itinerary or a letter from travel agents, airline, steamship company, confirming the purchase of tickets.

Health certificate issued by health authority that stated applicant is fit for travel and free from respiratory infection symptoms.

Diplomatic Visa

All diplomatic and service visa application submitted to the Consular Office of the Republic of Indonesia must enclose a diplomatic note from official agencies/missions/international organizations. The diplomatic note must include the following information concerning the principal applicant for all diplomatic and service visas, whether for temporary visits or assignments:
- Name and date of birth;
- Position and title;
- Place of assignment or visit;
- Purpose of travel;
- Brief description of duties;
- Travel date;
- Anticipated length of stay; and
The names, relationships, dates of birth, and travel dates of any dependents and other members of household who will be accompanying or joining the principal.
Health certificate issued by the health authority stating that applicant is fit to travel.
