• You are able to submit your most recent US passport
• Your expired US passport was issued within the past 15 years
• Your passport was issued when you were 16 years of age or older
  Your passport is running out of blank visa pages
  Your passport was issued over one year ago and you legally changed your name
• Your expired passport is not heavily damaged or mutilated

Submit your most recently issued passport.
The passport must have been issued less than 15 years ago 
The bearer must have reached their 16th birthday before the passport issue date.

Please send 1 recent professional passport type photograph.

The U.S. Passport Agency has introduced an online application form, this online style application must be used in all expediting cases no handwritten forms will be accepted by the U.S. Passport Agency. Failure to use this online application will cause delays in processing times.

Upon completion of the online DS-82, print the form and sign and date it.  It is advisable to sign the form in either blue or blank ink.

Letter MUST have an original signature; faxes, scans, photocopies, etc are NOT accepted by the United States Passport Agency. Click here to download the letter of authorization.

Copy of your airline or computer generated travel itinerary is required

If your passport was issued more than one year ago and you require an amendment such as a name change, you must submit the correct documentation required for a name change by marriage or court order.
All documents submitted for a name change or a data correction must bear the official seal of the issuing authority.

Photocopies and notarized documents are not accepted by the U.S. Passport Agency
