Tourist Visa
- Valid and signed Actual Passport , with at least 6 months from return date and with at least one blank visa page in your passport;
- One (1) photocopy of the passport data page;
One (1) Visa Application Form – Duly-accomplished Non-Immigrant Visa application form, typed or printed legibly in black or blue ink, and must be notarized;
One (1) colored photo, 2” x 2”, taken within six months before the date of application, showing a clear front view of applicant’s face, with a white background. No sleeveless attire. Blurred or low quality photos are not accepted. Passport Photo Guide – Click Here
Photocopy of latest bank statement and an employment certificate from the employer indicating position and salary, or affidavit of support;
Travel Itinerary – applicant must be a holder of a roundtrip/onward flight ticket out of the Philippines
For minor applicants – applicants (below 18) copy of birth certificate is required. If travelling on their own, affidavit of support and guarantee from either parent, as well as photocopy of parent’s bank statement shall also be required. For those 15 years old and below, a duly notarized Affidavit of Waiver for Exclusion Ground is required;

Business Visa

A letter from US employer or sponsor of the trip (click here for sample letter), indicating its specific purpose or nature and length of stay (inclusive dates) in the Philippines;
