Children under the age of 18) – must submit the following documents:
- Copy of the child’s birth certificate
- A Letter of Consent signed by both parents and notarized by a Notary Public.
- Copies of a Government issued ID such as passport or Driver’s License of both the parents should be attached.

A business letter (click here for sample letter) from US company – The letter must explain all of the following: Nature of business to be performed; Name and address of reference to be visited; This letter should request Multiple Entries visa if required; Guarantee of return transportation and sufficient funds for the visit;
The letter should be signed by an officer of the company other than the applicant.
Letter of invitation on letterhead from a host company or organization in Sierra Leone. Fax or scan is accepted.
